The National Transportation and Safety Board has issued a preliminary report on the crash of a medical helicopter in northern Iowa that killed three people.

The report indicates weather conditions were not ideal on January 2nd , 2013 when the Bell 407 helicopter went down in a farm field near Ventura. The helicopter from Mercy Medical Center in Mason City was on a trip to pick up a patient at the Palo Alto County Hospital in Emmetsburg, which is about 80 miles away.

Several witnesses say there were icy conditions at the time of the crash. A Mason City police officer traveling to the crash site reported his car slid through an intersection and other responders were warned of the slippery conditions.

The NTSB report does not speculate about the cause of the crash and the NTSB says its investigation is continuing.

Nurse Shelly Lair-Langenbau, paramedic Russell Piehl, and pilot Gene Grell were killed in the crash.
